‘I’ referred to as the recipient, one purchasing The Sacred Rose membership.

I understand that this is a consent form for accessing The Sacred Rose also known as ‘the membership’. The membership will host various forms of energy healing deemed fit by Carly Carreiro henceforth referred to as ‘Spiritual Mentor’. The energy healing modalities include, Chakra Balancing, Ahai, Sound Healing and Light Language.

I understand the following: that I should consult my physician or medical provider before attending the membership healing videos as energy healing modalities can have physical effects on the body. The community energy healing (especially sound therapy) can have effects on certain medical conditions such as diabetes, pregnancy and/or having metal in the body. Energy healing can affect insulin levels, and affect screw, pins and plates in the body. I understand I should consult my medical provider on any medical conditions.

I hereby give my consent to participate in this community and understand that the services provided by my chosen Spiritual Mentor are intended to provide relaxation and reduce stress. 

I understand completely that the services provided during this community are in no way a substitute for traditional medical treatment or advice. I am fully aware that my chosen Spiritual Mentor will not offer any diagnosis or recommend any Medical Treatment or Prescribed Medication. 

I understand that I must continue to have regular medical check-ups as part of my overall personal health care plan; and I should contact my own certified and licensed medical physician/doctor/health care professional for any physical or psychological ailments or concerns that I may have in order to get proper medical advice. 

I agree and understand that my participation in this community is voluntary and that at any time during the community I can choose to end my participation. I also understand that I may experience 'self-healing reactions' during the 48 hours following the energy healing sessions in this community.

I understand that any information exchanged during any session is educational in nature and is to be used at my own discretion, information shared in the membership is not medical, legal or financial advice.

Finally, I understand that by providing this informed consent I am assuming full responsibility for participating in this community and I hold harmless both my chosen Spiritual Mentor, Carly Spirit the facility/location where the services are provided.
